APLUS HEALTH CARE, LLC offers a wide range of services primarily to the adult who needs care in the home setting. Under the direction of the case manager, the Home Care team helps patients, families and caregivers manage acute illness, disability or long-term recovery needs. Patients, families and caregivers are encouraged to participate in planning and learning their care.
